5. Pick your Date and Time online.

We make it easy for you to choose the when to have your appointment.

After doing the above steps, look at the calendar and book your appointment online. Click on the various dates and you will see the available times for each date. When you are ready with the appropriate date and time, click on the black rectangle with your desired time to select that time. If you are viewing this using a cell phone, many patients tell us that the calendar is easier to view if you turn your screen to a horizontal position.

Also note that we will collect a $100 registration deposit. This deposit is refundable if you cancel your appointment before 24 hours in advance of your appointment. If you cancel your appointment within 24 hours of your appointment, or if you just don’t show up for your appointment, the deposit is not refundable. By booking online, you are consenting to receive messages (emails and texts) from our office regarding your appointment. This is how we communicate with you. If for any reason you don’t want to receive texts or emails, just call to explain, and we’ll figure something out. We respect your privacy.

Once you book your appointment, you will receive a link by email to fill out a form with your medical history. Make sure you do open that email and that you do complete the online medical history. We are very thorough in our practice, and we make sure we know the relevant details about our patients to do a good job.
